The image is a digital composite from the left side of the original shot.
In order to create perfect symmetry with the hands, the left side of the image was copied and flipped over the right side.
Lighting was a medium sized softbox place directly above the subject - allowing it to light the model as well as having some light spill onto the nuetral gray background.
