Make sure your tripod is perfectly levelled. You want your rows of photos to be aligned, not random. When shooting, let the overlapping between adjacent photos be about 30 percent. Camera should be set in full manual mode, manual focus. This is important if you want the panorama to be seamless.
Use Photoshop's panoramic stitching function called Photomerge. It's pretty self explanatory once to get to this part. Enable 'Fix Lens Distortion' and 'vignette removal' if necessary and let Photoshop do its magic.
